Flowline Health

We've had a strong month. Here are some highlights worth celebrating.

• One of our customers is coming out of pilot! And we’re seeing awesome results. Without saying too much, I especially loved this project because it involved us going deep into our customers’ operations, rethinking how they can leverage data alongside Voice AI, and delivering an agent that was very different from (but 10x better than) what we initially proposed.

  The project was admittedly harder than we expected, but the partnership with the customer and delivering an awesome solution in the end make it all worth it.

• We expanded a contract with an existing customer (by like 4x). To do so, we’ve built out an awesome new capability. Our agent can now navigate healthcare UIs in a HIPAA-compliant manner to handle all the button clicking, copying, and manual data entry work that would otherwise bore a human to tears.

Story of the week:

I was locked. Stranded in Mexico without my dopamine binky.

Have you heard of The Brick? It’s a small device that blocks you from accessing certain apps on your phone (Instagram, YouTube, and, unfortunately, LinkedIn for me). Once you’re locked, you can’t get into apps until you tap your phone to the brick.

Well, because of a poorly set timer, I accidentally bricked my phone while visiting my Dad in Mexico. Thousands of miles away from the device that unlocks it.

Upon realizing my mistake, my first thought was “OH NO! I can’t scroll Instagram until I get home.”

But then I realized: “I can’t scroll Instagram until I get home!”

The universe had given me the gift of focus.

Thought bugs:

Most people would rather stay in a job they hate than stomach the uncertainty of leaving.

Uncertainty is a tax the human mind hates to pay. This is largely due to what the late Daniel Kahneman called “loss aversion”, or the observation that humans weigh losses at roughly 2x the value of an equivalent gain.

i.e. losing 100 Euros feels far worse than gaining 100 Euros.

So when you’re wandering through uncertainty, the threat of the potential negative outcomes can loom over us much more than the excitement of potential wins.

But here’s the thing: no disproportionate gain will ever come without wandering through the mist of uncertainty. Its discomfort is not a warning sign; it’s the price of admission.

From New Year’s Eve in Thailand to a week of working remotely in Singapore, I had some rare time to disconnect, zoom out, and recenter on Flowline. More on that below.

About a month ago, I wrote that 2026 needs to be the year of focus.

One customer type. One offer. One wedge product our customers love so much they can’t imagine operating without it.

What I underestimated was how hard it is to fight momentum you’ve already created.

Relationships in the industry.
Offers you’ve already talked about publicly.
Customers asking for “just one more thing.”

Here’s a concrete example.

Last summer, Flowline’s first product was an agent that called patients after hospital discharge. We ultimately didn’t sell that version, but we did build a real network of hospital connections along the way.

Now we’re intentionally shifting toward smaller providers with simpler sales cycles — dermatologists, audiologists, CPAP distributors, and a few other niches we’re testing.

The problem is obvious in hindsight: the network we built to get here doesn’t actually overlap much with small, independent providers.

So I’m left with a necessary but slightly ego-bruising task: walking backwards and starting from scratch.

Cold emailing. Cold calling. DMs on LinkedIn.

Going back to the start can feel defeating, but it’s a test we need to run.

Story of the week

I’m a spy.

I’m in a Singaporean sauna listening to two men talk about real estate, raising money from family offices vs. non-institutional investors, and basically every other buzzword I irrationally enjoy.

I’ve invented a story for them: they both run their own funds, have been in the game for 20+ years, and are exactly the kind of people I’d want to accidentally overhear.

In my more tired moments, I think about Flowline as a “once this is done” game. Like a marathon I’m only running for the Instagram post and the right to eat an entire extra-large pizza afterward.

But listening to them reminded me: I don’t just like outcomes. I like the game!

Deals. Relationships. Money (even if people call it ugly).

The truth is, if we sold Flowline tomorrow, I’d still want to be the guy chatting about EBITDA in a sauna.

Manufacture luck.

In high school, I used to joke that my best friend Brian was lucky. Specifically because he won free tickets to three separate music festivals in a four-month span.

But I’ve been thinking more about what actually puts someone in a position to get lucky.

It’s surface area, and the shots that you take on goal.

Brian didn’t win those tickets because he was lucky. He won because he entered every radio sweepstakes in North America (and probably canvassed Europe for Tomorrowland too).

No one gets lucky living an actionless life.

I’ve developed a ritual: every time I write a newsletter, I need to go for a run first.

The run’s usually around Golden Gate Park. After I’ve sprinted a 5K, I’ll sit on a bench, babble into my phone, and somehow a newsletter emerges on the other side.

Why do I share this? Partially to brag (see my Strava for proof of a sub-8-minute mile 5K).

But also to highlight something important: the state in which you create matters almost as much as the will to create at all. When my blood is pumping, and my face is red, for some reason, the writer’s block lifts.

Flowline Heath

Despite living in San Francisco, I generally dislike the Y Combinator type of founder. If you’re not familiar with them, it’s the start-up bro with an overinflated sense of self and a tendency to look down on anyone not building B2B agentic workflows. Basically, all the worst parts of me.

But because I’m a big business nerd, I do religiously watch the Y Combinator YouTube videos.

One piece of advice they repeat: “Do things that don’t scale.”

And boy, we’ve been doing A LOT of that lately.

Since September, the demand for voice agents has been growing, and it’s clear that most medical administrative tasks, like appointment reminders, pharmacy calls, and even calling to collect email addresses, are in demand.

And if the customer wants it? We sell it, baby.

While the strategy was great for gaining traction, now we have to say no to all of the random use cases and start to focus.

Looking ahead to 2026, I’m convinced our growth will come from a narrow wedge. Not a shiny demo, but the ability to serve our customers better than any other provider.

Story of the Week

This week, I heard a story about two brothers. The younger is naturally talented at everything (flute, basketball, piano).

It drives the older brother crazy. He spends an hour and a half every night shooting hoops, just to be worse than the kid who barely tries.

It made me think about my own childhood. I wasn’t good at sports. I rarely hit my goals. Compared to my friends, I was consistently in second or third place.

I distinctly remember, in high school, hearing that my friends were applying to Stanford and thinking, “Dang, I’ll just be happy if I get a job in the insurance industry.”

But at 17, I learned to show up consistently for myself when I committed to losing weight (details in my post, Brushing Your Teeth).

That consistency was like learning how to put gas in my engine.

And when I found business (something I had a natural talent for), I realized I not only had gasoline, but a fast car to put it in.

So if I had to bet on one of those kids? I’d take the older brother.

What does the fear of vulnerability keep us from achieving?

When I told friends I quit my job, the reaction was usually, “Oh, cool. Good for you.”

But when the same friends saw me regularly posting on LinkedIn, the reaction was often, “OMG! I would never do that.”

Here’s the thing: being vulnerable costs a little embarrassment. Leaving my job could cost me hundreds of thousands of dollars. Our emotions are obviously very bad at gauging these risks.

And if we shiver at posting on LinkedIn, what else are we missing out on in life?

Flowline Health

We’re in the middle of an interesting deal.

We’ve had to totally reinvent our pricing model. The whitespace is fun, but I keep catching my lesser demons trying to slip in clauses that tilt things a little too far in Flowline’s favor.

But I think to myself: if the goal here is to work with this customer forever, does a sneaky clause that might screw them help me or hurt me?

Obviously, it hurts me.

I once heard that trust is the currency of business. Conceptually, I know this to be true, but it hits home in the moments where trust is actually built.

Here’s a thought exercise: In your personal life, when have you decided to trust someone?

Often, it’s when someone’s helped you or exposed themselves as vulnerable. They have shown a token of trust that you will not hurt them, and you return it in exchange.

So back to the deal.

I’ve made myself a rule: every time I send a contract, I’ll make sure the customer fully understands every line (pricing, minimums, refunds, everything).

I never want a scenario where a customer comes back and says, “Wait, I didn’t know we agreed to this.”

Why? Because life is easier when you trust each other. And what is a business if not making life easier for your customers.

Story of the week

Remember the ghostwriter I signed last newsletter? (Shout out to Morgan Snyder)

When we first signed the contract, I sent him this email.

I then proceeded to be high-touch.

Our calls are nothing like I expected. Rather than him creating content for me, the dynamic feels much more like a writer’s room / personal coaching session. And while I leave electrified, I was worried that my over-opinionated nature was a bother.

Until our most recent call.

As we were wrapping up he said, “This is really good, man. Most people get on these calls, and it’s like pulling teeth. But your input is helpful, it gives me stuff to work with.”

Turns out, my over-opinionated nature actually helped him do his job better.

AI Tools That Help Me

Some of you know I have severe dysgraphia. It’s like dyslexia, but for writing (I couldn’t spell ‘because’ until I was 9).

To help with this, I rely on two apps:

• WisprFlow: I speak into my computer, and it’s made me twice as fast.

• VoicePal: This is what I’m using to write this newsletter now.

Both tools give me a tremendous amount of leverage and time back every day.

So, here’s a confession: this weekend, I signed a ghostwriter.

I’ve realized I need to put myself out there on LinkedIn, but with limited time and resources, I’m bringing in a content guy to help keep me engaged and pumping out content (I also internally cringe a bit when I think about writing LinkedIn posts three times per week).

You might be wondering: “Wait, is Gabe going to stop writing this newsletter himself?”

Of course not! Writing this public journal is cathartic for me.

Anyway, enough about my content strategy. Let’s get to the real updates.

If you read my last update, you might have thought, “Wow, Gabe’s getting pretty big for his britches. Three customers and he’s already celebrating!”

Well, let me be clear: these britches are too big for me. In fact, I’m practically swimming in them!

The last two weeks have been a reminder of just how humbling sales cycles in healthcare can be. Deals I thought were at the finish line stalled, potential customers got busy, and I did a poor job keeping up the momentum.

My big lesson? The most important thing to optimize for is speed. After a demo, when the ROI is clear, I need to be better at keeping people focused and excited.

According to my little sister (a rockstar saleswoman at AWS), I need to build relationships outside of my champion so I can have multiple ways to drive deals down the field. Else, I might get stuffed at the goal line.

Story of the Week:

Exactly a year ago, I was staring at the ceiling of a hotel room at 2 a.m.

I was jetlagged, a bit hungover, and suffering from a horrid bout of insomnia. It was an odd moment to discover an internal truth: “I need to quit my job.”

For the last year and a half, my life was on a train I had never meant to board. I was on the tracks of big tech. Each stop meant working with kind and smart people, solving interesting problems, and racing to win one of the biggest infrastructure races in all of humanity (building AI data centers).

It was suffocating.

This feeling didn’t come from the job. It didn’t come from the people or the work-life balance (all of which were actually pretty great). It came from the truth that had just bubbled up to the surface.

If I never took the shot on myself, I would die a regretful man.

At this point, you might be thinking, “Wait, didn’t you quit 10 months later?” And in that, you’re right. What people don’t tell you is that knowing that truth doesn’t solve one central problem: it’s still really scary to quit.

But knowing that truth sparked action. I started setting goals. I began looking for ideas. I started offering to do side work. And most of all, I met my cofounder (shout out Andrew). So when I had my shot at Flowline, I took it.

This was something I heard from My First Million (terrible name, great podcast).

This hit home, especially when my daily habit tracker looks like this

It’s a reminder of two things:

1. I probably don’t need to track whether I took creatine every day.

2. If I don’t believe that I’m consistent and dependable, how can I expect others to?

It sure didn’t feel like it.

This journal’s ‘Flowline’ section is less about company progress, and more about my journey. Company updates will resume next issue.

We just landed our third pilot (let’s go), which gives me the inkling that we’re actually onto something.

This inkling is reinforced in instances where my girlfriend drags me out of my cave to a party or to socialize with friends. When I’m out, I hear things along the lines of:

“You already have customers? That was fast! Good for you.”

But those comments don’t acknowledge the months leading up to me quitting my job. For what seemed like forever, Andrew and I were going into the Healthcare Policy wilderness just to come up with horrible product ideas (one of which was cold-calling homeless people with AI. I wish I was joking).

It goes to show that when we reconnect with old friends to see they’ve lost weight, turned around their career, or are visibly more grounded, it’s easy to think that process was instantaneous.

But what we don’t see? It’s the uncertainty and doubt that comes with that journey before they start winning their first fights.

It reminds me of being 17, wandering around the gym in a 2XL t-shirt (pick up a dumbbell a few times, fail maxing out on bench. “This is what working out is, right?”).

Back then, a healthy body seemed so far away.

In retrospect, it wasn’t. Maybe 6 months. It only felt like forever because I didn’t know when being fat would end.

Story of the week.

“You’re covered! If you ever need to go to the hospital, just tell them that you’re insured by Medi-Cal.”

I couldn’t believe what I was reading from this chatbot.

For the last two months, I had been moving through the world assuming that I was completely untethered from health insurance (a bit ironic, given my company).

While I generally knew there could be a gap in my insurance coverage after I left my cushy Google job, what I didn’t realize was the underlying stress of knowing I was one accident away from being in crippling medical debt.

But by some grace of Gavin Newsom, I now find myself covered by California’s Medicaid, aka Medi-Cal.

Instantly, that slow current of stress evaporated. I was covered! None of these emergencies would tip me into destruction.

This small experience gave me empathy for the stress that everyday Americans have to face on a daily basis.

• First hard launch on social media!
  If you haven’t read it yet, check out Andrew’s article “Who Holds the Risk” (or at least throw it a like on LinkedIn).
  We’re still figuring out the balance between quality and engagement. I wanted to pair the launch with the meme below, but Andrew talked me out of it (doctors are serious people). What do you think: if I have to burn my LinkedIn by shamelessly promoting Flowline, should I post content like this?

• Project Snorlax
  Less than a month until patients get their hands on this! We’re meeting the team in person soon, and I can’t wait to shake our sponsor’s hand live.

• Project Charmander
  We’re rolling. The last few weeks were a doozie! More on this in the Stories section.

• The “Don’t Do Doctrine”
  I’ve been obsessed lately with the idea of constraints as a tool for creation. Each failed offer or target customer teaches you something to avoid. And when time is your most precious resource, it’s critical to capture those lessons and make them rules so you don’t make the same mistake twice.

  Andrew and I formalized this into a Don’t Do Doctrine. Instead of only writing down what works, we’ve created rules for what we absolutely won’t do.

  Examples:

  • “Flowline does not sell to hospitals unless we’re working directly with the sole decision maker.”

  • “Flowline will not deploy Flo in a scenario where she (or they?) makes clinical decisions.”

  We all set constraints in our personal lives (I don’t date men who have navy blue sheets, or I don’t eat pineapple on pizza). Why don’t we do it more often in business?

Story of the Week

“We’re worried. What we saw in the demo isn’t what we’re hearing now.”

My heart sank. The pilot customer we were celebrating last week was suddenly frustrated with us. In our eagerness to show how fast we could ship, we gave them an unpolished product.

After the meeting, Andrew and I stressed over every detail of how to explain this was fixable.

As I rewrote the “please don’t cancel” email for the 12th time, one phrase kept ringing in my head:

“Every problem in business is fundamentally a communication problem.” -I think Jesse Itzler said that

So instead of overthinking, we called them up. We put a label on every single thing that felt off with the Voice Agent (the voice sounded robotic, they cut me off to much, they didn’t know the phone number) and fixed them live, one by one.

By the end, they said: “I feel much better about these.” My heart sang.

I think about this constantly. I want more free time, more travel, less stress. But right now, that would mean fewer hours building this business, and not chasing my dreams. At the moment, I can’t pay that price.

Project Snorlax

We just passed IRB (Institutional Review Board) approval for our weight loss care companion! Since this is being piloted within a large academic medical center, the review process was stringent. All in all, it took about 2 months—which, believe it or not, is on the shorter side for these approval cycles. Patients will start using the tool in October!

Project Charmander

This is the big news I waited for. Thanks to an introduction from one of our all-star advisors (shoutout Stephanie Rogers), we signed a pilot with a telehealth provider to handle some of their non-clinical calls. Due to NDAs and respect for their business, I can’t go into too much detail, but this is a significant win for improving patients’ access to healthcare providers as they recover from hospital stays.

And remember that long sales cycle I mentioned in the last newsletter? Turns out, if you come with the right offer at the right time to an organization that moves fast, you can get things done. Even in a slow-moving industry like healthcare.

Working with this client completely redefined my understanding of how fast providers can move, and I’m thrilled to collaborate with them as we refine Flo for their use case.

Advisory Board

We’ve continued to expand our advisory board with a mix of permanent and interim advisors. Some highlights include:

• Stephanie Rogers, Chair of Readmissions at UCSF

• Corinna B. Clark, COO of Gardner Health Systems

• Luke Sorrell, Internist and PCP (and now longevity expert!)

• Pranav Gaurav, former Chief of Staff at Assort Health

Each brings a diverse background and network, and I’m incredibly grateful to lean on them and grow with them as we navigate this industry.

Story of the Week

I write this section because I believe that more than ideas and numbers, people connect with stories.

To back up this belief, I have a daily habit, “Homework for Life” (popularized by one of my intellectual role models, Matthew Dicks). It’s simple: I jot down a story from each day, and through reps and reflection, become a better storyteller.

Not all stories get retold, but I log them nonetheless. Here’s one from the past couple of weeks:

I recently found a flannel while passing a thrift store. From the moment I saw the shirt, I knew it was meant to be mine.

But a week later, I pulled my flannel from the dryer and realized I had shrunk the arms. I was devastated.

Laying in bed, I kept thinking, “This stinks. Guess I’ll just roll up the sleeves every time I wear it.”

But then another thought popped into my mind: “How do I expect to build a company if I’m the kind of person who wouldn’t even try to fix the shrunken arms on my favorite flannel?” Low agency. George Mack would be ashamed.

Ten minutes later, I was massaging conditioner into the sleeves and had built a contraption to gently stretch the fibers overnight (hint: it involved a clothing hanger, and a 16oz sweathshirt).

Thanks to my contraption and the belief that I can change my circumstances, I woke up to my favorite flannel restored.

P.S. This story is not about the flannel.

AI Side Builds

As a man selling AI solutions, I try to use it for most rote tasks in my life. For almost all of these, I use Lindy.ai (DM me for a refferal link baby!)

I always find it interesting to hear how others use their automations, so here’s a list of mine:

• My Workout Coach: This writes me workout plans every Sunday, ensuring progress each week with my weights and efforts. I used to pay for a trainer, but now that I’m balling on a budget, I created a system to write them for me.

• My Sales Coach: This goes through all my customer calls and emails me feedback on how I can improve. It’s helped me actively listen more, learn to quantify pain points, and get better at guiding questions.

• My Get Sh*t Done Agent: This is a personal favorite, despite the name being a bit raunchy. It creates a checklist for me to fill out every day, along with an interesting quote about life. Other than the custom quotes, there’s not really much AI incorporated. I just like having my checklists made for me.

Flowline Health

I’ve come to realize companies like Flowline (which sell to large healthcare systems) follow this trajectory:

Right now we’re on step 3: Getting a pilot.

We need to prove that our concept works, even if we charge zero to a fraction of the cost at first. This gives us grounds to go out afterwards and say “Look at this case study! Hospitals are saving time, money, and lives by calling their patients with AI. AND YOU CAN TOO!”

This however, is proving to be no small task. To get a demo with no track record requires building trust, selling the idea, and gaining buy in from stakeholders we don’t even know yet. Not to mention navigating the bureaucratic maze of large organizations.

But luckily, coming from the Mag7: navigating egos, and boosting internal metrics is no foreign concept to this guy!

Note: we’ve got some promising opportunities in the pipeline. It’s too early to share any publicly, but we’re excited :)

Story of the week

I went back to Google to return the last relics of my employment, and to drop something off to my favorite employee: the lady who works the coffee stand.

Her name’s Coty, and while I barely even know her, for the last 2 years she’s greeted me in the morning with a “Hi Gabe! How are you today?” For some reason, out of all the people in the building, I felt most compelled to leave her goodbye note.

When I started writing it I understood why. It read something like this:

When I moved to The Bay, I didn’t know it yet, but I was lonely. Hearing you say my name each morning meant so much more than you know.

When I handed her the note, I sprinted away. I couldn’t handle the vulnerability of seeing her read it (so instead I’m posting it on the internet!).

But the lesson rang in my head the rest of the day. To truly see someone else, even just saying their name, is of no cost to us. But who knows what it means to them.

Kindness is asymmetric.

This one comes from my (totally amazing) cofounder Andrew.

It was in regards to bringing on a partner to help us get intros for a pilot. But it prompted me to think of my actions as below:

You should do something if:

Cost (of time, money, etc.) < (potential value) x (probability of success)

Most of us know how valuable something is, and some of us are good at inferring cost. But the probability of success? That’s a dark art only good instincts and experience can bring you.

Gaining clout as a Substacker wasn’t my main intention (though it would be a nice side effect). I’m writing this to create a space where you can keep in touch and get to know me. The fact that so many of you decided to care is wild. Your support means more than you know.

Flowline Health

MVP Development:

We’ve built our MVP, a Voice AI agent that acts as a care coordinator for patients. It checks in to review medication, clarifies care plans, and generally replaces the stack of paper patients receive when leaving the hospital.

Project Snorlax:

We’re making progress on Project Snorlax, which uses our care companion technology for clinical weight loss programs. We’re partnering with a doctor who runs an obesity clinic, and as many of you know, this mission is near and dear to my heart. I struggled with weight throughout the bulk of my childhood, and if we can help even one person on their journey, I’d call this a success.

Advisory Board:

As most of you know, my background is not in medicine. To compensate for this, we’re building an advisory board of industry experts. We’ve already secured a few stellar candidates and are leveraging this function to fill gaps in knowledge, expand our network, and gain inroads into a notoriously walled garden of an industry.

Story of the week

I’ve been waking up stressed. From the expectations I’ve put on myself, the uncertainty of the outcomes, all of it.

So one day, as I was sitting in my car, I called my executive coach (aka a custom project I made in ChatGPT) to talk through this feeling. That conversation helped me realize I’d been focusing on the wrong things. It’s not the outcomes that define me, but the actions I take (was this thing trained on Tony Robbins???)

By the end of it, I landed on a corny, but true mantra: “I leave it all on the field.” It reminds me that my goal is to die knowing I relentlessly pursued my ideals and beliefs. Success is just a nice side effect.

Side note: Whenever I start doubting whether Voice AI belongs in healthcare, I think about this moment. If it can serve as a performance coach for me, it can probably help Grandma understand her medications.

The Tools Fallacy:
This one came from my friend Danny. The tools fallacy is the idea that we kid ourselves into thinking it’s the tools holding us back from taking action.

Like thinking you can’t start training for a triathlon until you’ve gotten goggles. But people have been swimming for thousands of years, and goggles have only been around for a few hundred. We can swim without them.
